You can get a fungus infection from a saline-filled implant.
Any known living fungi, virus or otherwise can not permeate the elastomer of an intact implant. That means in or out. The only thing that can permeate freely is (fresh) water [or lower molecular weighted silicones if it were a silicone-filled implant]. Even the salt in saline can not escape.
The molecules are just too big. Today implants are filled directly from a sterile bag of saline through a closed vacuum system. Secondly, according to the FDA nothing is supposed to be squirted inside the implant or pocket (e.g. Betadine solution) as it once was.
DEXTRAN was once used as a filler until is was discovered that it could sustain fungal life were a contamination to occur. By definition, DEXTRAN is a "polysaccharide consisting of glucose units linked predominantly in a-(1->6) mode. It is sometimes present in appreciable concentrations in sugar cane and mill products due to microbial activity."
Of course for saline filled implants, only sterile saline is utilized for a filler.
