Simply complete the application form and submit it. Your application will be processed within 24 hours, but usually the same day. You will be able to obtain status information online. Complete and accurate information will increase your chances of approval.

The patient must be the primary applicant. Immediate family member co-applicants are accepted. Applicants must be U.S. residents. Applications are processed between 7:00am to 5:00pm MST, Monday through Friday. Applications received after hours will be processed the following morning.
